Palatal expansion, also known as SARPE treatment (Surgically Assisted Rapid Palatal Expansion), is a surgically assisted rapid palatal expansion that is primarily used to treat malocclusions and narrow upper jaws. It is often used in adults or adolescents whose upper jaw growth is already complete, as conventional palatal expansion without surgical intervention is not sufficient for them.

Procedure for palatal expansion (SARPE)

Preparation:
First, the patient is examined by the orthodontist and x-rays are taken to determine the exact width of the upper jaw and to create the treatment plan.

Surgical procedure:
The actual procedure is performed by an oral surgeon under general anesthesia. The upper jaw is cut along certain lines (usually in the palate area and the maxillary sinuses). This makes the upper jaw more flexible to allow expansion through the palatal expansion apparatus.

Palatal brace:
A special palatal expansion appliance (expander) is attached to the upper jaw immediately after the operation or a few days later. This expander is gradually activated by the patient or the orthodontist to stretch the jaw. The expander is adjusted using a small key or screw and ensures that the two halves of the upper jaw are slowly pulled apart.

Expansion phase:
This phase usually lasts 2-4 weeks. During this time, the palate is gradually stretched so that the upper jaw becomes wider. This is done gradually to relieve the bone and allow for even expansion.

Stabilization phase:
After active expansion, the expander remains in the mouth for several months to stabilize the new position of the upper jaw and give the bone time to grow back together. This phase is important to prevent a relapse.

Aftercare and further treatment:
Successful palatal expansion can often be followed by orthodontic treatment with braces to bring the teeth into the correct position.

If the upper jaw is too narrow for the lower jaw, this can lead to problems with chewing and speaking.

A narrow upper jaw often leads to a crossbite, in which the upper teeth bite behind the lower teeth.

A narrow jaw can lead to dental crowding as there is not enough space for the teeth.
